Wellness Assessment Tearpad

I grabbed this for my office's wellness corner, and clients actually take them without asking. The tear-off pages are sturdy enough to write on with a pen without bleeding through, which makes them feel more legit than a flimsy handout.

The tear-off format makes it so easy to hand a sheet to a client during a session—no photocopying or cutting needed. Each page is laid out clearly enough that someone can fill it out in under five minutes, which keeps the momentum going. This has honestly become a staple in my weekly routine.
